ADMISSION REQUIREMENTS
The following shall be required before enrollment of child:
1. Registration Form – Completed and signed by custodial parents or guardian.

2. Immunization Record—Provided by child’s health care provider that documents and verifies that child has received current, ADHS recommended immunizations. It is the parent or guardian’s responsibility to assure that child receives these immunizations in accordance with ADHS-approved schedule along with a documented record from the health care provider  of such immunizations  as they are completed. Parents must provide this record upon enrollment for child to be accepted into the program.

3. Written authorization for release of child, including printed name of parents’ designee and designee’s signature. The center will not honor a phone authorization to release child without prior written authorization from parent.

4. Two weeks’ written notice is required to withdraw a child from the program. Kiddie Kampus
maintains the right to withdraw a child from the program if it is determined that his or her needs cannot be met. Verbal conferences will be held with parents to discuss problems before the parents is required to withdraw the child
form the program.

FEE SCHEDULE:
All Enrollments are on a full time basis.
One time Registration Fee—$45 (non refundable)
Full Payment is due on the Friday proceeding each week regardless of attendance. Tuition is considered late if not received in full by 6:00 p.m. Monday.
If late, a $5 additional fee will be assessed for that week.
A late charge of $1.00 per minute applies after 6 p.m.
